Terms & Conditions - the basics
1. Acceptance of Terms
By scheduling an appointment or using services provided by Medical Plug, you agree to be bound by these Terms and Conditions. If you do not agree, please do not use our services.
2. Services Provided
We offer mobile and on-site services for:

Phlebotomy (blood draws)

Specimen collection (urine, DNA, etc.)

Laboratory courier and delivery

Diagnostic testing coordination
All services are performed by trained and certified professionals in compliance with state and federal health regulations.
3. Booking and Payments

Appointments must be booked in advance through our website, phone, or text.

A non-refundable booking fee is required to secure your appointment.

Additional charges may apply based on the type of testing requested.

Testing and lab panel fees are billed separately and are not included in the collection service fee unless otherwise stated.
4. Refunds and Cancellations

Booking fees are non-refundable.

Cancellations made less than 24 hours before the scheduled appointment are subject to a cancellation fee.

If a technician is unable to collect a sample due to client refusal or inability to comply, no refund will be issued.
5. Specimen Handling and Responsibility

We follow strict protocols for specimen handling, labeling, and transport.

Once specimens are delivered to the testing laboratory, Medical Plug is not responsible for lab testing delays, lab errors, or result interpretation.
6. Confidentiality and Privacy
We are committed to protecting your personal health information in accordance with HIPAA and our Privacy Policy. Your data is stored securely and shared only with authorized entities as needed to complete your service.
7. Client Responsibilities

You must provide accurate personal and medical information.

You must remain still and follow instructions during sample collection to avoid complications.

If you are receiving DNA or legal testing, proper identification and consent may be required.
8. Mobile Service Limitations

Our mobile services are subject to availability and may be limited by weather, location access, or health safety restrictions.

If our staff feels unsafe at the appointment location, the visit may be canceled at our discretion with no refund.
9. Limitation of Liability
Medical Plug is not liable for:

Delayed results due to third-party labs

Misinterpretation of test results by the client

Any adverse reaction or complications following specimen collection
10. Governing Law
These terms shall be governed by and interpreted under the laws of the state of Tennessee
